Dr. Adam Holt

Dear Patient,

I hope this letter finds you happy and healthy. I am writing to inform you that my family and I have made a very difficult decision to leave Vermont and relocate to Virginia to be closer to our families.

Beginning April 1st, 2019 I will transition my role as your primary care dentist at Champlain Dental Group to Dr. Edward Nawotka or Dr. Robert Boyd. Your care at Champlain Dental Group will not change. I have the utmost trust and confidence that my colleagues will continue to provide you with outstanding dental care. Please disregard this letter if you are not a regular patient of mine.

It has been a privilege being your Dentist, gaining your trust and I am sad to say goodbye.

If you have any ongoing dental treatment, please know that I will be at Champlain Dental Group until June and am happy to take care of your dental needs.

I am grateful to you for entrusting me with your dental care over the years. Thank you for allowing me to be your dentist.

 Sincerely,

Adam Holt, DMD
